  • The applicant must be a resident of Uttarakhand.
  • The applicant must be a person with disabilities.

Application Process

Offline

To obtain a medical certificate, a person with disabilities must visit the district hospital on Wednesdays. They can collect a registration form there after paying a nominal fee. After registration, the individual is examined by the concerned doctors or specialists. The applicant must carry their Aadhaar card, mobile number, and a passport-size photograph. The Chief Medical Officer (CMO) issues the disability certificate based on the medical examination reports. If not available immediately, it can be collected later from the CMO office.
