Eligibility

  • Resident of Madhya Pradesh.
  • Applicant must be a family member/dependent of the deceased.
  • Deceased was the primary breadwinner.
  • Family must be Below Poverty Line (BPL).
  • Deceased age at death: 18 to 60 years.

Application Process (Offline)

  • Step 1-2: Get the form from Gram Panchayat or Public Service Center and fill it.
  • Step 3-5: Submit with documents, receive acknowledgment, and wait for verification by the District/Gram Panchayat office.
  • Once verified, the applicant is added to the monthly pension proposal.

Documents Required

  • Aadhaar Card and Photo.
  • Voter ID or Driving License.
  • Death Certificate of the deceased.
  • Proof of Age of the deceased.
  • BPL Card / Ration Card.
  • Bank Passbook.
