The scheme “Family Planning Indemnity Scheme” was launched by the Directorate of Health Services, Government of Goa. The scheme aims to provide compensation for death, failure, or complications following sterilization operations, addressing the financial burden and risks associated with family planning procedures.

Benefits

  • Section A: Death following sterilization (inclusive of death during sterilization operation) in hospital or within 7 days from discharge – ₹4,00,000/-.
  • Section B: Death following sterilization within 8-30 days from discharge – ₹1,00,000/-.
  • Section C: Failure of sterilization – ₹60,000/-.
  • Section D: Cost of treatment in hospital and up to 60 days arising out of complications following sterilization operation – Actual cost not exceeding ₹50,000/-.

Conditions

  • The claimant must accept the compensation amount as full settlement with no further claim rights.
  • The claimant must warrant the truth of all particulars provided; false statements lead to forfeiture.
  • Complications, failure, or death must be reported within specified timeframes.

Eligibility

For Beneficiary:

  • Must have undergone sterilization operation under family planning program.
  • Must have undergone the operation at an authorized health facility.
  • Must provide documentary evidence of the sterilization procedure.

For Claimant (Family Member/Legal Representative):

  • Must be a legal claimant or family member of the deceased or incapacitated person.
  • Must have legal authority to represent the beneficiary.

Application Process

Offline:

Step 1: Initial Reporting and Documentation
Report the case to the Medical Superintendent or Health Officer of the facility where sterilization was conducted. Collect original sterilization certificates and case papers.

Step 2: Form Completion and Medical Certification
Complete Part A of the Claim Form cum Medical Certificate. Submit all records to the Chief Medical Officer at the district level for medical certification (Part B).

Step 3: Application Submission
Submit the medically certified form with original documents to the respective health facility.

Step 4: Follow-up and Verification
The District Indemnity Scheme Committee will review the case.

Step 5: Payment Processing
Verified cases are forwarded to the State Health Society for final approval. Compensation is disbursed directly to the bank account.

  • Consent Form cum Application Form for Sterilization Operation.
  • Original Sterilization Certificate and Ligation Certificate.
  • New Case Paper, Admit Paper, and Discharge Card.
  • Aadhaar Card and Election Card (Husband and Wife).
  • Birth Certificate of Children.
  • Bank Details (with 2 photocopies).
  • Death Certificate (in case of death).
  • Original bills and prescriptions for complication treatment.
