To facilitate and take medical services to the next level, the State of Goa envisaged the present scheme to provide its people with quality medical care and treatment of diseases, hospitalization, and surgery through government and private hospitals.

Benefits

Benefits are provided on a cashless basis up to the annual limit:

• Family of 3 or fewer members: Insurance cover up to ₹2,50,000/- per annum.
• Family of 4 or more members: Insurance cover up to ₹4,00,000/- per annum.
• The benefit can be availed individually or collectively.
• Covers 447 medical procedures.

Eligibility

• The applicant must be a resident of Goa.
• Minimum continuous residence in Goa of five years or more.

Application Process

Online / Offline

• Approach an Empanelled Health Care Provider (EHCP).
• After doctor consultation, if the procedure is covered, approach the DDSSY Sahayak at the hospital with your card.
• The Sahayak will raise a request for pre-authorization.

Documents Required

• Aadhar Card/Enrollment Slip for every family member (above 5 years).
• Residence Proof: Passport, residence certificate, Election card, or Driving license (issued min. 5 years ago).
• OBC/SC/ST: Caste and Income certificates (income < ₹6,00,000 for OBC).
• Differently abled: Disability certificate.
• Ration card (if unavailable, provide Marriage/Birth/School certificates and parent dependency declaration).
